Appointments for New Patients

If you are a new patient, the following steps must be taken prior to being seen by a clinician:

  1. Before printing our new patient paperwork, call the office and verify that your insurance is accepted by our practice; if it is, we will obtain your basic demographic information and create an electronic medical record for you.
  2. You can then acquire our new patient paperwork by requesting it via mail or by printing it here.
  3. Fill out the paperwork and mail or fax it to us and we will contact you to schedule your first appointment.

Note: It is our policy that your initial appointment with Shawsheen Medical be with a clinician other than Dr. Tenney, while any subsequent appointments may be scheduled with her.


Shawsheen Medical Associates offers several different appointment types including:
• Preventative/Screening Examination
• Follow-up appointment
• Preoperative Clearance
• Same day sick appointment

When you are sick, please call us first so that we may coordinate your care and so that we can arrange a timely appointment in the right location. When it is appropriate, we will make a same day sick appointment for you to be seen in our office. We also may coordinate your care with urgent care/walk-in centers for issues like lacerations, orthopedics, trauma. Additionally, we work with all of the tertiary hospitals, but we are most closely affiliated with Lawrence General Hospital and Beth Israel Deaconess Medical Center.

There are many types of appointments, including routine/preventive or screening exams, routine follow-up urgent or same day sick appointments and pre-operative medical clearance appointments. When you call, state the reasons(s) for the visit, so that we schedule the appointment in a timely manner. If you are ill, call first thing in the morning and we usually can see you the same day that you call. We can do routine EKG and spirometry testing, we have a lab for drawing blood and collecting urine specimens. However, we do not have on-site Xray and we do not do minor surgical procedures including suturing. Thus, when you call we will see you if we can offer what you need, or we may direct you to an urgent care or emergency room.


No Show Policy:

We do our very best to accommodate all our patient appointment requests. We require a minimum of 24 hours notice to cancel or reschedule appointments and 48-hour notice to cancel or reschedule physical appointment, so we can offer that time slot to another patient.

Canceling or rescheduling appointments without 24-hour notice (or 48-hour notice for physicals) will result in a fee.

Cancellation fee for an appointment without the 24-hour notice is $25. Fee for canceling or reschedule physical appointment without 48-hour notice is $50.


Appointment Policies:

In order to be seen by a physician or nurse practitioner, you must call the office and make an appointment. Walk-ins are strongly discouraged.

Please arrive at least 15 minutes prior to your scheduled physical appointment.

Canceling appointments without a 24-hours notice will result in a fee.

Patients arriving late for the appointment may need to be rescheduled to another date, time and or provider. When you are late,  we will try to offer you another appointment on the same day, but depending on the schedules and fairness to the other patients, you may have to wait to be seen.

Patients are given courtesy reminder calls at least 48 hours prior to their scheduled appointments.

Patient insurance information is reconfirmed at each appointment. Demographic information is updated yearly. Please notify the office of any address, phone number or insurance changes prior to your appointment.

For identity protection, those patients 18 and older, a photo ID is required and will be scanned into the computer.


Referral Policies:

All referrals require five (5) business days to process. You must contact your insurance carrier prior to making any appointments with a specialist in order to confirm that provider is part of your network.

Referral requests should be made prior to being seen at the specialist.  Remember this is a request for a referral not a guarantee that it is approved.

Shawsheen Medical Associates is affiliated with Lawrence General Hospital. All referrals and specialist appointments will be confirmed with doctors within our network. All visits must be pre-approved by your PCP. Any visits with specialist not within our network, may not be allowed and the cost of that appointment can and may be your own financial responsibility.

Call the office to request the referral, please have the following information when calling:

Name of the specialist (address, phone and fax number)

NPI number

Date of your appointment

Reason for the visit


Insurance Policies:

Always bring your insurance card and co-payment to all your appointments.

Reconfirm with your insurance that your procedure and or visit is covered in your plan for that date.

As we are part of Lawrence General Hospital network, we accept most insurance. Please check with our billing department for more information.
